What is the process to getting started?

  • Contact our office by sending an email to admin@phforward.com or use the Contact Us tab at the top of the page.

  • If it’s your first time meeting with us, there will be paperwork for you to fill out prior to the start of your appointment. A link will be sent to the email address you provided so that all necessary documentation can be completed prior to your appointment.

  • If your appointment is virtual, you will need to login to your account in order to access your private meeting space through a secure online portal.

  • Intake: $225

  • Follow-up: $140

Reunification Therapy

  • Self-Pay only

  • $250/hour

Medication Management Rates

  • Intake (60 minutes): $325

  • Follow-up (40 minutes): $195

  • Follow-up (30 minutes): $170

  • Follow-up (20 minutes): $140

ADHD Testing

  • 60 minutes (computerized test & results)

  • Insurance may cover part, or all, of the service; prior authorization may be required

  • Self-pay cost: $325

Psychological Diagnostic Testing

  • Self-pay only

If you would like to use your insurance, please provide your information (name, date of birth, carrier, member ID, and insurance phone number) or a copy of your insurance card.
